Tell us what happened, how it affected you and what you would like us to consider. Raising a concern should not affect fair access to care.

Raising a concern for someone else

You may raise a concern about your own experience or ask about acting as an authorised representative. Appropriate permission and confidentiality checks are needed before information about another person can be discussed.

An initial email should ask for the appropriate secure route if your concern involves health information or clinical records. Do not attach these to ordinary email.

Contact about a concern

Do not include health information or records in ordinary email. For immediate danger or urgent health needs, use emergency or NHS services rather than waiting for a complaints response.
